PHR Diabetes Mentorship Program - frozen

The mission of the PHR Diabetes Mentorship Program is to educate the patients of free clinics like Jericho Road on diabetes and diabetes management through medication, diet, and lifestyle changes. In doing so, the program will also provide experience for the students of the Jacobs School of Medicine in lifestyle management, working with vulnerable populations, and patient education, as well as an opportunity to gain service hours.
